2-80 CLIENT STATISTICALDATA

POLICY
The Agency will collect and store the following client statistical information:
- referral sources,
- number of clients referred,
- applicants not accepted with reasons,
- number of clients registered,
- number of clients served by age, gender, program and place of residence,
- presenting problems,
- number of clients assessed,
- number of treatment/action plans developed,
- services provided,
- clients failing to complete the programs,
- number of clients whose service was terminated,
- number of clients who completed the program,
- number of discharge and aftercare plans.
PROCEDURE
The Agency uses the Client Services Manager database (CSM-DB) to electronically collect client statistical data for the purpose of program planning, evaluation and Ministry reporting requirements.
Service Directors will maintain client statistical data for their respective programs.
