Program Description

Concurrent Disorders Case Management is a short-term (up to 4 months) intensive case management program for individuals struggling with both mental health and substance abuse issues.  The Case Manager conducts a strength-based assessment and develops a treatment plan in collaboration with the client to address housing needs and provides support, linkages and referrals to psychiatric services, residential substance abuse treatment and long-term supports.

Mental Health Case Management is a short-term (up to 4 months) intensive case management program for individuals with mental health issues who are being released from provincial custody.  The Case Manager conducts a strength-based assessment and develops a treatment plan in collaboration with the client to address housing needs and provides support, linkages and referrals to psychiatric services and long-term supports.

Hours of Operation:  Monday to Friday 8:30 a.m. to 5:00 p.m. Evening appointments are available.
