ANNUAL STAFF DECLARATION POLICY It is essential that the Agency ensure that staff and volunteers remain capable of performing their duties and responsibilities throughout their time with the Agency. PROCEDURE As part of the annual performance evaluation, staff will be required to complete a St. Leonard’s Community Services Annual Staff Declaration form that indicates his/her continued ability to perform the duties and responsibilities of his/her position with the Agency. This declaration is part of the Performance Evaluation form and is contained in Appendix 10 Staff Supervision Model Addendum 3 of the Human Resources Policy.
