Appendix 26 ACCREDITATION TEAM - TERMS OF REFERENCE
Approved January 2009
- MANDATE
To lead the Agency in the process of achieving and maintaining CMHO accreditation.

COMPOSITION
The team will consist of:
- Executive Director (Chair)
- Director of Children’s Residential Services
- Director of Addiction & Mental Health Services (Accreditation Co-ordinator)
- Manager - Justice Services
- Director of Employment Services
- Board Member
- Executive Assistant (Secretary)
- Human Resources Administrator

TERM
Until accreditation is achieved on or about May 2009.

QUORUM
A quorum will consist of not less than 5 members

DECISION MAKING
All decisions will be determined by consensus.

SCHEDULE
The team will meet monthly with the exception of July, August and December.

AUTHORITY
The Accreditation Team is a standing committee of St. Leonard’s Community Services reporting to the Management Team.

ROLES AND RESPONSIBILITIES
- To determine program areas eligible for accreditation.
- To develop a work plan.
- To undertake a self-evaluation study.
- To promote and facilitate participation of all levels of organization.
- To liaise with CMHO accreditation group.
- To co-ordinate site reviews.
- To respond to the site review report.
- To communicate with staff on progress.
- To develop a plan to ensure continued compliance.
- The Chair of the team is to notify the Human Resources Administrator of any changes to the membership.
