6-390 BENEFITS

POLICY
The Agency conforms to Federal and Provincial mandatory provision of benefits and remits the required premiums on behalf of staff from the date they commence employment. In addition, the Agency provides additional coverage through plans with an outside carrier plus Agency sponsored benefits.
PROCEDURE
GOVERNMENT BENEFITS:
Canada Pension, Employment Insurance, Workplace Safety and Insurance Board (WSIB) and Employer Health Tax.
REGISTERED RETIREMENT SAVINGS PLAN
All staff are entitled to participate in a voluntary R.R.S.P. payroll deduction plan administered by the Agency.
REGISTERED PENSION PLAN
The Agency administers a contributory registered pension plan whereby the Agency matches the staff’s contribution based on a percentage of gross earnings. Participation in the plan is mandatory for all permanent full time staff upon completion of the three month probationary period. Part time staff are eligible to participate after two years of continuous employment working at least 700 hours in each of the two consecutive calendar years.
EMPLOYEE ASSISTANCE PLAN (EAP)
All staff and/or eligible dependents are entitled to participate in the Agency E.A.P., wherein the Agency funds the first three visits of the fiscal year and 50% of the balance to a maximum of 10 visits per family. Staff participating in EAP as a result of abuse allegations made by a client will be entitled to 100% coverage by the Agency.
GROUP INSURANCE BENEFITS
Permanent full time staff will receive the following additional benefits upon successful completion of his/her three month probation period. The three month period will be waived for staff who have worked at least three months in a temporary full time position, prior to being offered permanent full time status.
Accidental Death and Dismemberment, Long Term Disability, Life Insurance, Dental, Extended Health Care and Medical Emergency Travel Insurance benefits are provided at the Agency’s expense for all permanent full time staff and their eligible dependents who have passed the three month probationary period.
Group insurance benefits will continue for one year from the date of injury for staff on a WSIB claim as per government regulations.
Group insurance benefits will continue for staff on an unpaid leave of absence due to illness until such time as the staff qualifies for Long Term Disability (LTD) or is terminated.
Group insurance benefits for staff who qualify for LTD will continue until the staff is terminated.
Group insurance benefits with the exception of LTD coverage for staff on an unpaid leave of absence for personal reasons will cease at the end of the month that the leave started. LTD coverage will be suspended as of the day the leave starts.
VISION CARE
The Agency will reimburse for vision care expenses, prescription glasses or contact lenses and eye examinations for permanent full time staff and eligible dependents up to a maximum of $300 per family annually upon presentation of valid receipts. Costs reimbursed will be considered a taxable benefit to the recipient.
AGENCY SPONSORED BENEFITS:
STUDY DAY - All staff are entitled to one paid study day per college/university course, per semester, for courses that are required or subsidized by the Agency. Study days do not apply to field placements, practicums, co-ops or project courses if they do not involve class time or written examinations.
SMOKING CESSATION PLAN – All staff are eligible to participate in the Agency’s Smoking Cessation Plan.
The Agency will pay the cost of a quit smoking aid (i.e. the patch or gum), up to a maximum of $120, upon presentation of valid receipts, see Appendix 16 Smoking Cessation Plan Record Sheet. Staff who utilize this benefit will be requested to complete a survey regarding his/her progress at one month, three months, six months and one year.
This benefit is not available to family members. Funds paid through the plan will be considered a taxable benefit to the recipient.
FITNESS CLUB MEMBERSHIP - The Agency will reimburse up to 50% of the cost of an annual fitness club membership to a maximum of $300 or 50% of the purchase of home fitness equipment to a maximum of $300 per year for permanent full and part time staff. Some local examples include Athletic Club, YM-YWCA, McFit and The Fitness Centre.
Examples of eligible home equipment reimbursements include:
- Cardio equipment such as elliptical, treadmill, exercise bike, rowing machine, stepper, bow-flex
- Free weights and benches, dumbbells, barbells
- exercise DVD's/videos
- Yoga/pilates supplies such as exercise balls, mats, tension bands
- Wii Health/Fitness games such as Fit, Jillian Michaels Fitness Ultimatum, Gold's Gym Cardio Workout, My Fitness Coach, The Biggest Loser, Active Life Outdoor Challenge
- Outdoor bicycles
There are two options for reimbursement:
- The staff will sign up and pay for the annual membership or purchase home fitness equipment and forward a valid receipt to be reimbursed by the Agency. The reimbursement will be 50% of the cost of the annual membership or purchase to a maximum of $300 per year.
- The staff will sign a contract for an annual membership to be paid by monthly instalments. For the first month payment, the staff will forward a copy of the contract plus proof of commitment to pay monthly for 12 months. Staff will be reimbursed for 50% of the monthly cost to a maximum of $17.00 per month for 12 months. Payments will be included with the regular payroll direct deposit.
Only staff memberships will be reimbursed, this does not apply to other family members. Fitness club memberships for periods less than a year will not qualify for reimbursement. Costs reimbursed will be considered a taxable benefit to the recipient.
