6-200 STAFF IDENTIFICATION CARDS
Approved November 1999

POLICY

Identification cards will be issued to all staff, volunteers and students within the first week of employment.  Identification cards must be replaced every five years. 

PROCEDURE

Staff, at the discretion of the Service Director, may be issued business cards stating his/her name and position with the Agency.

All I.D. cards will be returned to the Agency for destruction upon request or on the date of the staff's termination.
