The following notice is posted in an easily accessible and highly visible location within each of St. Leonard’s Community Services’ locations.

Appendix 1 - Notice to Clients of St. Leonard’s Community Services

          Personal Health Information Protection Act (PHIPA)

The privacy of your personal health information is important to all of us at St. Leonard’s Community Services.

The PHIPA legislation creates rules for the collection, use and sharing of personal health information that is needed by healthcare providers including Counsellors (Primary Workers), doctors, social workers, psychologists and other designated health information custodians.

At the time of intake into our program, St. Leonard’s Community Services staff will provide you with an explanation of our policies and procedures that govern how we store, manage and share your personal health information.

You have the right to request access to your own client file information from our programs through your Counsellor (Primary Worker).  You also have the right to request corrections to your client file if the information is inaccurate.

You have the right to determine how your personal health information can be used.  St. Leonard’s Community Services staff share information from you or about you with your informed consent, and for specific purposes agreed to by you except where required by law to do otherwise.  These instances are expressly permitted by PHIPA and may include compliance with some statutes such as Child and Family Services Act, Coroners Act and Vital Statistics Act.

St. Leonard’s Community Services has policies and procedures that set out the length of time that client records will be kept and the security of those records.  It complies with applicable laws governing the retention of information.
