Concurrent Disorders Outreach Team
The Interdisciplinary Concurrent Disorders Outreach Team provides multi-disciplinary services through a formal partnership between St. Leonard’s Community Services and the Brant Community Health Care System. Rotational onsite services are available at key community partner locations including: Towards Recovery Clinic, Winston Court (Brantford Housing), Brantford General Hospital, etc.
A priority population for this service includes mothers with co-occurring opioid addiction and mental health issues impacting their daily functioning in several life domains, and individuals with complex conditions who are unable to access traditionally offered office based services due to barriers (i.e. stigma, child care, transportation, income or housing security, literacy).
Intensive services are provided through multiple treatment modalities including individual, group, family and peer support interventions by an interdisciplinary team comprised of the following team members: Concurrent Disorders Counsellor, Family Intervention Specialist, Consultative Occupational Therapist, Peer Support Worker, Art Therapist, and Registered Nurse. Individuals must require intensive intervention from at least two members of the interdisciplinary team. A formal diagnosis from a regulated health professional is not required, as the eligibility focuses on impairments in adaptive functioning in several life domains/roles. Family intervention services are available to family members whose loved one meets the above program criteria.
